Below are several links to forms that a Northrise University Student will need on a regular basis.

  • Academic Document Collection Form

    To be used when students or their designated parties are collecting academic and other documents from Northrise University.

  • Application for Student Benefit Form

    To be used by students who are applying for a benefit provided for by a Northrise University policy.

  • Change of Payment Plan Request Form

    To be used by students who wish to apply for a Change of Payment Plan after the SMS Registration Record has been approved and an initial payment has not been made.

  • Course Exemption Form

    To be used by students who wish to be exempted from a course that they have already completed and meets the standards of the Exemption Policy as outlined in the student handbook.

  • Course Drop Add Form

    To be used by students who wish to drop or add a course.

  • Course Withdrawal Form

    To be used by students who wish to withdraw from a course.

  • Donor Writing Class Registration

    To be used by Northrise University students who are required to attend the Donor Writing Class as students currently on Financial Aid or Students who wish to apply for FA.

  • Exam Appeal Form

    This form should be completed if you are requesting the re-grading of a Final Exam.

  • Exam Resit Form

    This form must be completed by students who are applying to re-sit an exam

  • Graduand (Graduating Student) Profile Submission Form

    This form is used by potential Graduands to submit their profile information.

  • Graduation Application Form

    In order to be awarded a certificate in your field of study, graduating students must fill in this form.

  • Graduation Gown Purchase Application Form

    This form is to be filled in by Graduands who wish to purchase a Graduation gown.

  • Independent Study Application Form

    This form is to be filled in by students who intend to engage in a self-directed or Independent Study (IS) plan of study.

  • NEW STUDENTS: Financial Aid (FA) Application Form

    This form is to be completed by new students who wish to apply for Financial Aid.

  • Nursing Student Clearance Form

    This form is to be completed by Bachelor of Science in Nursing students who have completed the program and passed the General Nursing Council licensure examinations.

  • NU Student Contact Details Update Form

    This form is to be filled in by students when updating their contact details.

  • Online Learning Student Checklist

    This form is to be filled in by ALL NU students at the beginning of a Registration Period to assess readiness for online learning.

  • Request for Time Away from NU Accommodation Form

    The form is filled in by students who wish to request for Time-Away from NU Accommodation during the Semester.

  • Research Project Extension Form

    The form is filled in by students who wish to request for a Project/Research extension.

  • Request to Change Program/Study Mode

    This form should be completed by students who wish to change their program major from the program they were initially admitted into. The form should also be used by students who wish to change Study Group/Mode.

  • Resident Assistant Application Form

    This form is to be completed by a student wishing to serve as a resident assistant for the University Hostel. 

  • RETURNING STUDENT: Financial Aid Application Form

    This form is to be completed by returning students who wish to apply for Financial Aid.

  • Student Council Application Form

    This form is to be completed by a student wishing to contest for a published vacant position in the student council.

  • Transcript Correction Form

    This form should be completed if you are requesting for a correction on your official transcript from the University. The form must be accompanied by the stipulated processing fee as indicated in University documentation for the current academic Year.

  • Transcript Request Form

    This form should be completed if you are requesting for an official transcript from the University.

APA (American Psychological Association) style is mostly used to cite sources within the social sciences among the many writing styles available to academicians. Northrise University adopted the APA writing style as the official style that all students must use in their academic writing. For more information, please consult the Publication Manual of the American Psychological Association, (7th ed.) as the latest writing style. This policy has been designed to establish the principles and processes that govern the academic writing style progress for all programs offered at Northrise University. Academic progression is the process by which a student can advance in their program of study by meeting the academic and administrative requirements for that program.

Work-Study Policy

  • DAY students receiving Financial Aid (FA) are required to participate in Work-Study
  • You will be assigned an area to work throughout the Semester

Student Responsibilities

  • Your first priority is education

  • Doing your part in Learning Teams for 3 & 4th year
  • Maintaining the highest ethical standard in your interaction with faculty, staff and fellow students

Student Conduct

You are not allowed to:

  • Present other people’s work as your own (plagiarism)
  • Alter or falsify University documents, records
  • Disrupt the educational, administrative processes or other campus functions
  • Abuse (physically or otherwise) on or off campus any member of the University community (or anyone else for that matter)
  • Steal campus property or steal from members of the University community
  • Damage campus property
  • Possess or sell illegal drugs, alcohol and other behavior-altering substances
  • Possess dangerous items or deadly weapons
  • Engage in lewd, indecent, or disorderly behavior
  • Provide false information about the University
  • Solicit or assist others to act in any of the activities addressed above.
